Any good layouts for a food journal?

I have to keep track of all the food I eat for a college class. Are their any layouts that you know of? I do not need to have the calories or fat, just the foods/drinks themselves. Any suggestions?

I usually draw lines down my page to make 4 columns. Then put a label at the top of each:
Date Time Food/Beverage Amount

I write foods in blue and beverages in green so I can tell the difference. The time helps me see if there is a time of day where I tend to splurge and the amount helps me keep track to make sure I am sticking to a balanced diet and include all of those nibbles/sips you take through the day! It is amazing how a bite of a cookie, 1 fry and a sip of the latte add up!
